      Governance/Infection Control/Prevention of Radicalisation
    • Enhanced DBS Certificate – Issued last 2 Years if not Subscribed to the DBS Update Service – Copy still required if on DBS update service
    • Certificate of Completion of Training (CCT/JCPTGP) Certificate
    • Smartcard Number
    • Document Evidence of Immunity against Hep B – Antibodies (Titre Levels are mandatory)
    • NHS Email Address
    • Performers List letter - Stating you are a GP Performer on the Performers List
